We have received drawings for the above project. We are unable to complete the review of the drawings
until the following information is provided:
1. Submit a copy of the menu or a listing of all of food and beverage items to be offered at this
food facility.
2. Are there any openable windows?
3. Submit a sample of the epoxy flooring/base and the manufacturers' specification sheets.
4. Show the finish material for the wall pass shelf (item #48) and accumulator casing/counter (item
#125).
5. Show all floor sinks and floor drains on the revised plumbing drawings.
6. All floor sinks must be accessible for cleaning. If the floor sink is located within a self -serve
display cabinet, provide a minimum 4 -inch, 3/8 -inch radius continuously coved base surrounding
the floor sink.
7. Provide hot water (minimum temperature, 120°F) and cold water to kettle and pot filler assembly
(item #71) and to Reel-Kleen Retractable Hose Reel and Spray (item #137) through a mixing
faucet equipped with a backflow prevention device.
8. Provide a slope of at lease 1:50 (1/4 -inch per foot) to the floor drain near the item #71 and item
#137.: Area depressions are not approved.
9. Provide an additional hand sink with permanently mounted soap and paper towel dispensers,
. near item #18 in the "prep area".
10. Submit hot water demand calculations for all fixtures.
11. Show the location of the water heater on the revised drawings.
12. Submit the completed Exhaust Ventilation Systems form for all Type I and Type II hoods. See
enclosure.
13. Sneeze guards must intercept a direct line between the consumer's mouth and clean plates and
utensils, as well as displayed food. End panels may be required on the sneeze guards.
14. Provide a sneeze guard at the self -serve food display, (item #100).
15. Sheet FS -1 shows a condiment table. How will the condiments be dispensed?

Until the information requested is received, the drawings are considered to be disapproved.
Applications for which no approval is granted within 180 days following the date of application shall
expire by limitation, and drawings and other data submitted for review may thereafter be returned to the
applicant or destroyed by this department. This department may extend this period for an additional 60
days upon written request by the applicant showing that circumstances beyond the control of the
applicant prevented further activity. In the former case, the applicant must resubmit plans and pay a new
plan check fee.
