$268.00. The fee is due with this request form and will not be refunded if
the request is denied.

Dear City of Cupertino:
Evershine Vl, LP the owner of Marketplace Shopping Center has changed the address for
19744 Stevens Creek Boulevard, so we would like to have the address recognized as
19748 Stevens Creek Boulevard. Everything about the space remains the same (location
and square footage), so we have reflected the new tenant's lease with the address as
19748 Stevens Creek Boulevard. There are no other tenants between 19748 Steven
Creek Boulevard and Longs that has the address of 19750 Stevens Creek Boulevard (see
Sattached).
