City of Cupertino Orange Ave. Sidewalk Closure Project.
Dear Ms. Michael:
4LEAF, Inc. (“4LEAF”) appreciates the opportunity to submit our Cost Proposal to provide Project
Management services in support of final closeout activities for the City of Cupertino (“City”) Orange Ave.
Project. The original Service Order No. 1 that was issued to 4LEAF for the Orange Ave. project has been
closed by the City and a new Service Order under our existing On-Call Construction Management (CM)
and Inspection agreement with the City is needed to assist with the Orange Ave. tasks outlined below.
At the completion of Project’s field activities in November 2022, PG&E had yet to relocate two power
poles that were located within the footprint of Orange Ave. PG&E has since installed new poles and
moved their electrical wires to the new poles; however, AT&T and Comcast have yet to relocate their
lines to the new poles. Comcast and AT&T have indicated they are still evaluating if they need to develop
design drawings in order to move their lines.
4LEAF perform as the City’s point of contact for general communication and coordination with AT&T and
Comcast on moving their lines to the new poles; attend any site visits that require the City’s input to
AT&T, Comcast, or the Public; and assist the City with responding as necessary to public concerns from
service disruptions when AT&T and Comcast move their lines to the new poles.
Coordination of the above activities will be performed by Chris Bryden who was 4LEAF’s on-site
representative during the Orange Ave. project. Our estimated time and material, not to exceed costs to
perform these tasks is Four Thousand, Two Hundred Dollars ($4,200).1
