ARBORIST REPORT
Mr. David Wong
RE: 21733 Alcazar Avenue.
Cupertino CA 95014
TREE INFORMATION
Common Name: Coast Live Oak
Botanical Name: Quercus agrifolia
Height of Tree: 30 feet
Diameter of tree at 48 inches above the natural grade: 19 inches
Circumference of tree at 48 inches above natural grade: 60 inches
NATURE OF WORK:
Request permission to remove a dead Coast Live Oak Tree.
CONDITION OF TREE: This tree is affected by Pacific flatheaded borer, this beetles attacked this oak tree.
The crown is in evident decline. Boring damage is typically characterized by cracking and loosening of
the bark and 3/16 -inch oval exit holes. Frass is retained within galleries. At the trunk base I saw Inonotus
dryophilus this fungus is associated with decline and advanced wood decay. I found Fruit bodies of
Annulohypoxylon thouarsianum, a wood rooting pathogen are commonly associated with advanced
wood decay especially common on Quercus agrifolia (coast live oak). The decayed wood caused by these
fungi leads to tree failure or limbs failure.
REASONS FOR REMOVAL: The property owner, Mr. Wong wishes to remove this Oak tree, as he is fearful
that the tree collapse, and damage his property. After the recent failure limb impacted his house. The
Oak Tree is located on the west side yard, nine feet from the house and 1.5 feet from the property fence
ARBORIST RECOMMENDATION: I believe this tree qualifies for an exemption from protection under the
private tree by law. This tree is, in my opinion, structurally and physiologically unsound, as
demonstrated by severe beetle attack, decayed wood, tree is Dead, potentially High hazard for failure
branches or tree. I don't recommend plant a new tree in same spot, the spot is small , the new tree will
encroach with the fence , roots will destroy the fence, the canopy spread interfere with house or
neighbor's house, below this condition Mr. Wong would the fee for don't replacement tree.

Tree Removal Permit Application
Dead tree removal and replacement permit application:
I am requesting a permit to remove an oak tree on my property which has died. The
crown of the tree turned brown this past winter and did not recover in the spring Within
the last month, the tree dropped a branch, which pushed me to request the removal
application. I am afraid the tree has become a hazard now.
Included in this permit submittal is a report from an ISA certified arborist on the
condition of the tree.
I am electing to pay an in -lieu fee, instead of planting a replacement tree. I do not feel
there is a location on my property where the replacement tree will be suitable.
• The same location of the dead tree in the side yard is too narrow, so the canopy
interferes with the house. I have it trimmed every 2-3 years.
• The front yard is dominated by a large 50ft redwood tree.
• The east side planting strip adjacent to Berry Ct, has PG&E lines running under it.
• The back yard has two 30ft oak trees. These also require trimming every 2-3 years to
keep them back from the house.
Sincerely,
David Wong
