As a condition of approval, the facility could be renewed pending a review of the technology to determine if the visual impact of the equipment could be further minimized. Pursuant to this condition, staff conducted a site visit and determined that the visual impact of the equipment could be further minimized by installation of needle socks that fully wrap the antennas (the existing condition only covers the face), replacing the needled branches with ones that are more realistically colored, and by more seamlessly integrating the lower two rows of branches with the base of the tree. Furthermore, as conditioned, exposed electrical wires and units installed on the tree that are not covered in needle socks, are to be painted green to match the foliage of the treepole. 2 DIR-2013-28 10121 Miller Avenue 2 ; , 9 Sp.ti: <r: ,` ,il i ,, . , , ,: . , _, :: ; pk ' " . :. , . - . ; "w . , . . ' „, i ` ,. . , , , , ,, . i ; , ., , -,:,, ,, , a . ,: ,.._ , - _' , , V w ,. Horizontal and vertical antenna patterns are combined for modeling purposes to accurately reflect the energy two meters above ground level where on-axis energy refers to maximum energy two meters above the ground along the azimuth of the antenna and where area energy refers to the maximum energy anywhere two meters above the ground regardless of the antenna azimuth, accounting for cumulative energy from multiple antennas for the carrier and frequency range indicated; and That the Occupational Safety and Health Administration has policies in place which address worker safety in and around communications sites, thus individual companies will be responsible for their employees' training regarding Radio Frequency Safety. In summary,it is stated here that the proposed operation at the site would not result in exposure of the Public to excessive levels of radio-frequency energy as defined in the FCC Rules and Regulations, specifically 47 CFR 1.1307 and that T-Mobile's proposed operation is completely compliant. Finally,it is stated that access to the tower should be restricted to communication industry professionals, and approved contractor personnel trained in radio-frequency safety; and that the instant analysis addresses exposure levels at two meters above ground level and does not address exposure levels on the tower, or in the immediate proximity of the antennas. 2 of 5 'Se 111 Crown Castle BU: 827839 SF922 Richwood Site Summary Carrier Area Maximum Percentage MPE T-Mobile 209 00 T-Mobile 0.515°/a Composite Site MPE: 1.724% 3 of 5 ,Se T-Mobile SF922 Richwood Carrier Summary Frequency: 2100 MHz Maximum Permissible Exposure(MPE): 1000 NW/cm^2 Maximum power density at ground level: 12.09208 NW/cm^2 Highest percentage of Maximum Permissible Exposure: 1.20921 % On Acis Area Max Power Max Power Height Orientation Density Percent of Density Percent of Antenna Make Model feet de rees true ERP atts NW/cm^2 MPE (NW/cm^2 MPE Ericsson AIR 33 B4A 62P 51 340 1601 2.623259 0.262326 5.123288 0.512329 Ericsson AIR 21 B4A B2P 51 340 1477 2.758938 0.275894 3.776041 0.377604 Ericsson AIR 33 64A 62P 51 100 1601 2.623259 0.262326 5.123287 0.512329 Ericsson AIR 21 B4A B2P 51 100 1477 2.758938 0.275894 3.776041 0.377604 Ericsson AIR 33 64A 62P 51 220 1601 2.623259 0.262326 5.123287 0.512329 Ericsson AIR 21 B4A 62P 51 220 1477 2.758938 0.275894 3.776041 0.377604 4 of 5 lsire T-Mobile SF922 Richwood Carrier Summary Frequency: 1900 MHz Maximum Permissible Exposure(MPE): 1000 NW/cm^2 Maximum power density at ground level: 5.14579 NW/cm^2 Highest percentage of Maximum Permissible Exposure: 0.51458 % On Axis Area Max Power Max Power Height Orientation Density Percent of Density Percent of Antenna Make Model feet de rees true ERP atts NW/cm^2 MPE NW/cm^2 MPE Ericsson AIR 21 62A 64P 51 340 687 1.283225 0.128323 1.756296 0.17563 Ericsson AIR 21 B2A B4P 51 340 687 1.283225 0.128323 1.756296 0.17563 Ericsson AIR 21 62A 64P 51 100 687 1.283225 0.128323 1.756296 0.17563 Ericsson AIR 21 62A B4P 51 100 687 1.283225 0.128323 1.756296 0.17563 Ericsson AIR 21 B2A B4P 51 220 687 1.283225 0.128323 1.756296 0.17563 Ericsson AIR 21 62A 64P 51 220 687 1.283225 0.128323 1.756296 0.17563 5 of 5 lsire SITE INFORMATION APPLICANT SITE I.D.#: 827839 TOWER CONTACT: CROWN CASTLE GT COMPANY LLC 510 CASTILLO STREEf STE.302 SANTA BARBARA,CA 93101 JOHN DOHM,AICP 825-957-1629 APPLICANT: CROWN CASTLE GT COMPANY LLC 510 CASTILLO STREEf STE.302 SANTA BARBARA,CA 93101 82957-1629 PROPERTY OWNER: D&D REAL ESTATE LP 10121 MILLER AVENUE STE.200 CUPERTINO,CA 95014 APN: 369-07-0O1 SITE ADDRESS: 10121 MILLER AVENUE CUPERTINO,CA 95014 COUNTY: SANTA CLARA 20NING: HEART OF THE CITY SPECIHC PLAN AREA ZONING JURISDICTION: SANTA CLARA SIGNATURE BLOCK APPROVAL SIGNATURE DATE Projecf Manager Consfivcfion Manager RF Engineer Sife Acquisifion o Conwlfant Property Owner 7-MObie RepresentaNve LE M N PINE R WN C AS T O C O CROWN CASTLE BUN 827839 10121 MILLER AVENUE CUPERTINO, A 95014 SF BAY MARKET VICINITY MAP " `', ertir.9 fet t;lsa z' , J's ;„ u. c7'f e , x3raits ' feitl ,.,4r7;;1, „ Eila r^r'rusci ",e ' i,w. c_ LfilPSiin y'' ,_ c' 2' ;C!F rr'li?{;f ,., .. ° ° f PROp,.ECT = u AKCA i',rk r-'r;:,.-ti: cta:«,-.!"'rr.,.['+I+;r; Ct:aertine .1"Vir,%tiPi _ i {>rtlP ; irll ' E,aton Elnnt3v Y: 5 c.hrrrai " _1 , „ ti =- tr '"li,J,, .x .. :4 „r'4,r. ', ,rrfi w `.s,:, yy.e rfi?;If: ,;crrero Safew w iitpt flsl;i r,a,snz7i c-,gQ rrk y '-" Secqwir,c E(errrtrrY `i;ti,rsf ?t,`r.i;,yer r4c1 DRIVING DIRECTIONS FROM SAN FRANCISCO INTERNATIONAL AIRPORT +%d.', I ¢._„ ;Vl i n,w;r I iir}lf:=,[jr,Le, ry II G ...
