APPROVED PROTECT This approval is based on a letter confuming a site inspection of the subject tree by Arborist Gil Mitchell(#WE-0134A), dated July 27,2012 and the Tree Replacement Planting Plan. 2. TREE REPLACEMENT The applicant shall plant one 24-inch box Raywood Ash and one 24-inch box swamp myrtle trees as replacements in accordance with 14.18.190 Table A (Replacement Tree Guidelines). These trees shall be planted according to the tree replacement planting plan, dated Apri115, 2013. 3. NOTICE OF FEES DEDICATIONS. RESERVATIONS OR OTHER EXACTIONS T'he Conditions of Project Approval set forth herein may include certain fees, dedication requirements, reservation requirements, and other exactions. Pursuant to Government Code Section 66020(d) (1), these Conditions constitute written notice of a statement of the amount of such fees, and a description of the dedications, reservations, and other exactions. You are hereby further notified that the 90-day approval period in which you may protest these fees, dedications, reservations, and other exactions, pursuant to Government Code Section 66020(a), has begun. If you fail to file a protest within this 90-day period complying with all of the requirements of Section 66020, you will be legally barred from later challenging such exactions. Staff has made at least one of the findings necessary to grant the tree removal permit for the dead tree and prescribed the removal and replacement in accordance with Section 14.18.120B (Exemptions) and 14.18.190 Table A (Replacement Tree Guidelines) of the Protected Trees Ordinance. Box 320819 Los Gatos, CA 95032 Re: White alder tree at Woodspring Condominium Association Dear Ms.Colla, At your request, I evaluated one white alder(Alnus rhombifolia)tree located at the above referenced association on July 23�d of this year. This letter serves as a summary of my evaluation. The evaluation was done from the ground and the tree was not climbed. A photograph and a site map are enclosed with this letter. 4 Woodspring Condominium Association is located at 22330 Homestead Road in Cupertino. The white alder tree is located at the front of the property and adjacent to Homestead Road. The trunk of the tree measures 21.5 inches in diameter at a height of four and one half feet above ground line. I inspected around the base of the tree and there is evidence of oak root fungus(Armillaria mellea)disease and of phytophthora (Phytophthora sp.J disease. The tree has virtually no foliage. There are fruiting bodies of mistletoe (Phoradendron serotinum)in the upper canopy but these fruiting bodies did not kill the tree. The tree has died from a combination of oak root fungus and phytophthora diseases that killed the vascular system of the tree at and just below the soil surface. I recommend removing the tree as soon as possible.
