Ladies and gentlemen:
As requested, Krazan & Associates, Inc. has provided Special Inspection services for the subject
project. Our services, performed on November 4, 2011, consisted of observing the completed
installation of post -installed anchor bolts in concrete and the performance of torque tests to
confirm proper installation torque. The special inspection services provided were in accordance
with the requirements of the City of Cupertino, as stipulated on the approved plans and in the
California Building Code. A copy of our field report documenting our observations and test
results is attached.
CONCLUSIONS
Based on the observations of our technician and the results of our tests conducted at the site, it is
our professional opinion that the anchor bolts installed on November 4, 2011 were, to the best of
our knowledge, installed in accordance with the approved project plans, the requirements of the
California Building Code and the manufacturer's installation procedures.
LIMITATIONS
Construction inspection and testing (special inspection) services are techniques used to check
construction for conformance with the project requirements as the work progresses and to reduce
the potential that construction elements are not in compliance with the project requirements as
the work is completed. Even with diligent construction monitoring, observation and/or testing
some construction defects may be missed. Performance of construction observation and/or
testing services (including but not limited to special inspection services) by Krazan &
Associates' personnel is not insurance, nor does it guarantee construction of any type. In all
cases, the contractor retains sole responsibility for the quality of work, for adhering to plans and
specifications and for repairing defects, deficiencies or omissions, regardless of when they are
found.

We have performed our services in a manner consistent with the standard of care and skill
ordinarily exercised by firms of our type practicing under similar conditions at this same time and
locality. However, we do not undertake the guarantee of construction or production of a
completed project conforming to the project plans and specifications. No warranty or guarantee
is expressed or implied. Our services during the identified period of time were limited to those
documented in our attached report.
