1. BID SUBMISSION: The City uses a two-part Bid process with Bids in Envelope "A" and State-
ments of Qualification and l lnanciat information in Envelope "B", City will accept Emrelape "A"
and Envelope "B" no later than Z O6 p m on July 21, 2015 in the Cdy Clerks Ofrce. Bids will be
publicly read at 2:15 p,m. in the City Hall Conference Room C located at 10300 Torre Avenue.

3. STATEMENT OF QUALIFICATIONS: Each Bidder shall be required to submit a "Statement of
Qualifications" in accordance with Document 00200 (Instructions to Bidders) and Document
00450 (Statement of Qualifications for Construction Work).
4. DESCRIPTION OF THE WORK: The work under this contract includes extending the left turn
pocket at Stevens Creek Blvd and Perimeter Rd as per the plans and specifications. All work is
to be done on the westbound Stevens Creek Blvd left turn pocket at the Perimeter Rd intersec-
tion. Modifications consist of removing existing landscapes and trees in the median, dearin0
and grubbing, grading, cutting back islands, installation and compaction of new AC paving and
base rock, installation of new curb and gutter, rerouting of utilities, and all general conditions,
general requirements and incidentals required to complete the Work in its entirety.
5. ENGINEER'S ESTIMATE: The Engineer's estimate is $3,7,094AQ,
6. CONTRACT TIME: Work shall be completed within 45 Calendar Days from the date when
Contract Time commences to run.
7. REQUIRED CONTRACTOR'S LICENSE: A California Class "A" General Engineering Contractor
or Class "C-12" Earthwork and Paving Contractor license is required to Bid this contract.
Joint ventures must secure a joint venture license prior to award of this Contract. Removal,
handling, and/or disposal of hazardous materials may, .by law, require hazardous substance re-
moval certification by the Contractors State License Board.
8. REQUIRED CONTRACTOR AND SUBCONTRACTOR REGISTRATION rAppEes if bid is submitted on
or alter Mardi 1, 2015 or if Contract is awarded on or after Ap 1, 2016. Labor Code Section

A.Owner shall accept Bids only from Bidders that (along with all Subcontractors listed in Docu-
ment 00430 Subcontractor List) are currently registered: and qualified to perform public work
pursuantto Labor Code Section 17255.
B,Subject to Labor Code Sections 1771.1(c) and (d), any Bid not complying with paragraph 1.01A
above shall be returned and not considered; provided that if Bidder is a joint venture (Business
& Professions Code Section 7029.1) or if federal funds are involved in the Contract (Labor Code
Section 1771.1(a)), owner may accept a non -complying Bid provided that Bidder and all listed
Subcontractors are registered at the time of Contract award.
9. PREVAILING WAGE LAWS: The successful Bidder must comply with all prevailing wage laws
applicable to the Project, and related requirements contained in the Contract Documents.
10. INSTRUCTIONS: Bidders shall refer to Document 00200 (Instructions to Bidders) for required
documents and items to be submitted in sealed envelopes to City Clerk's office, as well as ap-
plicable times for submission.
11. SUBSTITUTION OF SECURITIES: City will permit successful BidrIV, to substittrt� sec_:'flies fcr
retention monies withheld to ensure performance of Contract, as set forth in Document 00680
(Escrow Agreement for Security Deposits in Lieu of Retention), in accordance with California
Public Contract Code, Section 22300. By this reference, Document 00680 (Escrow Agreement for
Security Deposits in Lieu of Retention) is incorporated in full in this Document 00100.
12. PRE -BID CONFERENCE: No pre -bid conference will be held for this project.
13. PROCUREMENT OF BIDDING DOCUMENTS: To obtain a copy of the bid documents, Bid-

15. RESERVATION OF RIGHTS: City specifically reserves the right, in its sole discretion, to reject
any or all Bids, or re -bid, or to waive inconsequential deviations from Bid requirements not in-
volving time, price, or quality of the Work.
16. PORTION OF WORK PERFORMED BY THE CONTRACTOR: The Contractor shall perform no less
than 33% of the Work, excluding Specialty Items, with its own organization. Specialty Items will
be identified on the Bid Form.

3.STATEMENT OF QUALIFICATIONS: Each Bidder shall be required to
submit a "Statement of qualifications" in accordance with Document
00200 (Instructions to Bidders) and Document 00450 (Statement of
Qualifications for crossing Guard Services).
4.1DESCREPTION OF THE WORK: Crossing guard contractor is to provide
fully trained and equipped adult crossing guards, during designated
times and at specific locations, throughout the City of Cupertino. The
contractor shall administer and coordinate the program meeting the
requirements of the latest edition of the Manual on Uniform Traffic
Control Devices Calffornia Supplement: Section 7E.02 and as detailed in
the 2015 Contractual Crossing Guard Service - rebid contract docu-
ments.
5.CONTRACT TIME: The term of this agreement shall be three (3) years
from the start date of the agreement. The City shall retain the option
to extend the term of the agreement for an additional two (2) years, for
a possible total of five (5) years.
6.REQUIRED CONTRACTOR`S LICENSE: A California Contractors License

7.PREVAILING WAGE LAWS., The successful Bidder is not required to
comply with all prevailing wage laws applicable to the Project

substitute securities for retention monies withheld to ensure perform-
ance of Contract, as set forth in Document 00680 (Escrow Agreement
for Security Deposits in Lieu of Retention), in accordance with Califor•
nia Public Contract Code, Section 22300. By this reference, Document
00680 (Escrow Agreement for Security Deposits in Lieu of Retention) is
incorporated in full in this Document 00100.
11.PROCUREMENT Of BIDDING DOCUMENTS: Bidders may examine a

To obtain a copy of the bid documents. Bidders must download a set of

site that carries them. The City will post all addendums to the project
on the City web site and it is the Contractor's sole responsibility to
download the addendums for the project If a Contractor wishes to be

formation: Contractor's name, address, phone and fax number, an e-
mail address forfuture communication. The plan holders Eistwill be up-
