Event Description
1.Please fill out the attached Cupertino Special Event Permit Application and Fire
Department Event Permit.

have been uploaded to ProjectDox.
2.Please indicate if this event is for employees only or open to the public.
•This event is for employees and their invited guest.
3.Please provide the date of each of the events and the exact beginning and ending
times.
•Event will take place December 2 - December 17, 2025, Monday through Saturday,
6:00 PM - 10:00 PM
4.Please provide a site plan showing any tents or stages or ramps, or any other
structures or play structures that will be installed. Please show on your plans any
electrical equipment, lighting, heaters or generators. Please label each tent, booth or
activity area with the specific activity that will occur there.

5.Please also provide the product information for any structures or electrical equipment.
Will there be any outdoor cooking, grilling or fire pits?
•See event package for product information, uploaded to ProjectDox. There will not
be any outdoor cooking, grilling or fire pits.
6.The information you filled out in Accela indicates that alcohol will be served and it
states that you have obtained a temporary license from the Department of Alcohol and
Beverage Control. Did you contact them, and did they require a one-day alcohol sales
and service permit?
•No temporary license needed as this is a private event.
7.Will there be any food trucks or other outside vendors? If so, please provide a list.

•There will be no food trucks or other outside vendors.
8.The fee amount has been invoiced in Accela (see the attached invoice). You may either
pay the fee online on the ACA Portal with a credit card (however a 3.4% credit card fee
will apply) or you may bring in a check to City Hall for the amount and then no credit
card fee will apply.
•Invoice received. I will pay the fee online.
