WHEREAS, on Tuesday, June Thirtieth, Two Thousand and Nine, the City of Cupertino wishes to recognize _____________; and
WHEREAS, we admire her commitment to truly putting forth the maximum effort of saving ____________’s life; and
WHEREAS, she assisted a dog that was in need and showed great compassion towards him/her; and
WHEREAS, she recognizes animals as sentient beings, capable of suffering and experiencing pain; and
WHEREAS, she effectively dealt with multiple priorities protecting the life and health of all babies and dogs involved in the event.
NOW, therefore, be it resolved, that the Mayor and City Council of Cupertino, do hereby commend and recognize

for her outstanding willingness and effort to come to the aid of ________ and saving his/her life.
IN WITNESS THEREOF, I have hereunto set my hand and caused the seal of the City of Cupertino to be affixed this Tuesday, June Thirtieth, Two Thousand and Nine.
