WHEREAS, on Saturday, September Twenty-Fourth, Two Thousand and Sixteen, the City
of Cupertino wishes to recognize The Forum on the occasion of their 25th Anniversary;
and
WHEREAS, the Forum has more than four hundred residents, employs over three
hundred people, and offers Residential Senior Living, Assisted Living, Skilled Nursing
Care, and Rehabilitative Services on its 54-acre campus; and
WHEREAS, the Forum also hosts an annual Resource and Information Health Fair
attended by more than 600 people that is open to the greater community and offers health
screenings, fitness demonstrations, healthy food samples and more than fifty exhibits
from non-profit organizations and other groups; and
WHEREAS, the Forum collaborates with area hospitals, physicians, nurses, and geriatric
specialists to host seminars and training for professional in the senior living field; and
WHEREAS, the Forum maintains a Five-Star rated Health Care Center that has provided
Assisted Living, Skilled Nursing and Rehabilitative services to thousands of Forums
residents and seniors within the greater community over the years.
NOW, therefore, be it resolved, that the Mayor and City Council of Cupertino, do hereby
commend and recognize
The Forum
for being an innovative leader in the senior living industry and for establishing a Wellness
Center and Program that is a model for other senior living communities.
IN WITNESS THEREOF, I have hereunto set my hand and caused the seal of the City of
Cupertino to be affixed this Saturday, September Twenty-Fourth, Two Thousand and
Sixteen.
