A RESOIIJTION OF THE CITY OOUNCIL OF THE CITY OF aJPERI'INO
AUI'HORIZING EXEaJTION OF AGREEMENT WI'IH GATES MCOONAlD
FOR AI:MINISTRATION OF THE IDRKERS CXJMPENSATION PROGRAM
WHEREAS, Gates McDonald has provided administrative services to the
City of CUpertino since 1979; and
WHEREAS, the City Council of the city of CUpertino approves continued
administrative service for the City of CUpertino's Workers' Compensation
Program;
NCM, THEREFORE, BE IT RESOLVED that the City Council of the City of
CUpertino hereby authorizes execution of the contract (Exhibit nAn) with
Gates McDonald,

CONTRACT AMENDMENT
Effective October 1, 1990, the annual fee for Workers' Compensation
Self-Insurance services shall be as follows:
$9,000 billed in equal monthly installments of $750.00. Service fees applicable
to contract periods which are less than a calendar month shall be prorated. Invoices
are issued at the beginning of each calendar month and are payable within thirty
(30) days from date of issuance.
The above amendment to the contract effective September 4, 1979 is accepted

In the event this service contract is discontinued, Gates McDonald will return
all of the open files at the time of termination for an additional fee to be agreed
upon at that time.
