Article II, Paragraph J, of our Agreement for State Administration of Local Sales and Use Taxes provides that the city agrees to furnish two maps of the annexed area with appropriate addresses for city sales and use tax purposes. Annexations are processed by this Department to identity any business in the area so that your city will receive the local sales and use tax collected. Please send the following: 1. Two (2) maps with street address inserted showing beginning and ending street numbers in the annexed area. (See sample attached) 2. Two (2) locator or vicinity maps which show the annexed area in relation to city boundries. 3. A complete alphabetical list of all streets in the annexation showing beginning and ending street numbers. If a street is on the border of the annexation, indicate if the odd or even numbers are in the annexed area. (See sample attached) 4. If the territory is undeveloped, a statement to that effect should be placed on the map.
