Assignment
San Jose Tree Service has been contracted after a large oak had fallen in the front yard of the
property. An assessment was performed on the reason for the failure of the fallen oak and an
assessment was performed on the standing oaks on site. The following report will provide the
observations of the assessment. The assessment was performed by Certified Arborist Robert Apolinar
#WE-8846A.
Observations:
The trees on site appear to be in the rang of 50-80 years of age.The landscape and possibly home
appear to have been built around the trees. There are 4 large oak trees on site and the remaining trees
are young and still small. The site is on a sloped lot and there are teered planting beds to flatten areas.
The property is well kept and maintained. There does not appear to have been any major landscape
construction within the last 5-10 years.

Pruning History:
The tree had been recently pruned.
Leaves and twigs:
The leaves and twigs appear to be healthy.
Crown and branches:
The crown leans heavily over the pool area.
Trunk:
The lower trunk has been infected with Armillaria root rot and the bark is beginning to fall off in several
areas. Mycelial fans are present underneath the bark and the white rot has spread throughout the
wood. The wood is already beginning to deteriorate and loose structural stability.Approximately 80% of
the root crown has been infected. The tree has completely failed and has damaged significant areas of
the landscape. The failure was due to the root rot. A description of the root rot will be provided at the
end of this report.
Recommendation:
The tree has already failed and has been removed. The tree will be replaced with a species approved by
the city of Cupertino.

Roots and root crown:
The roots and root crown are in poor condition and pose a major hazard to the residents. Armillaria root
rot has been located in the lower root crown and major roots. The fungus has progressed and the
majority of the wood in the root crown has been affected. He major roots and root crown have major
rot as a result. The tree is in the process of completely dying due to the root damage. At this stage the
tree is losing its structural integrity and should be removed as soon as possible.
Health Assessment:
The tree is in poor condition due to the root and trunk damage. The tree is in the process of completely
dying and should be removed as soon as possible.
Recommendation:
The tree should be removed as soon as possible due to the raid failure of the root system. There is no
cure for the disease and the tree is liable to fall at as it progresses.

Pruning History:
The tree had been recently pruned.
Leaves and twigs:
The leaves and twigs appear to be healthy.
Crown and branches:
The crown leans heavily over the pool area and is unbalanced.
Trunk:
The main trunk has several large cavities that were filled with concrete in the past. The trunk leans
heavily towards the neighbor's property and should it fail, it may cause damage to both properties. The
root crown was also buried due to the landscape installation in the past. As a result, the trunk has been
infected with Armillaria root rot and the bark is beginning to fall off in several areas. Mycelial fans are

Health Assessment:
The tree is in poor condition due to the root and trunk damage. The tree is in the process of completely
dying and should be removed as soon as possible.
Recommendation:
The tree should be removed as soon as possible to prevent further damage to the landscape should the
tree fail. There is no cure for the disease and it is likely to continue spreading due to the trees condition.
Tree#4: Quercus agrifolia
DBH:42"
Height:40-50 ft
Spread: 30-40 ft
Pruning History:
The tree had been recently pruned.
Leaves and twigs:
The leaves and twigs appear to be healthy.
Crown and branches:
The crown is balanced.
Roots and root crown:
There was a very small area of decay located in the lower root crown. The damage is minimal and should
not affect the tree stability or health at this time. It should be monitored annually to assess condition.
Recommendation:
No service is needed for the tree at this time. The tree should be monitored and water should be kept
from the root crown in the summer months.
Armillaria description
Armillaria root rot is commonly recognized by the presence of light brown mushrooms (known as
"honey mushrooms" due to their color; not their taste!),which typically appear in a cluster of several to
dozens of mushrooms at the base of infected trees or shrubs. Sometimes the mushroom cluster will
arise from a buried root in a turfgrass or groundcover area away from the infected tree (or where an

Damage:
The first and most obvious symptom of Armillaria root rot may be decline or death of a tree or woody
plant. Often, death occurs so rapidly that the leaves turn brown, but don't yet fall off the tree.
Sometimes this decline may be slower, characterized by dieback of shoot tips over the course of months
or even years.Armillaria infects woody plants and destroys their vascular cambium and the underlying
root wood.
After the cambium has been killed and the underlying root wood has been invaded, a mycelial fan
usually grows to colonize the main stem, where it may continue to invade other roots or move up and
around the root collar. Where the cambium of the main stem or large roots has been destroyed by the
fungus, the formation of new wood and bark ceases, and a flat lesion on the surface of the bark(visible
as an external canker) develops—these are the best places to look for mycelial fans under the bark,
especially on trees that have canopy symptoms.
