We did not inspect the interior of finished walls or behind
installed finished cabinet work. We did not inspect the areas
immediately under furniture or appliances. Thin structure
contains slab flooring, and in slab floor type construction, it is
possible for Subterranean Termite infestations to be concealed and
not evident at the time of our inspection; therefore, we assume no
liability for any infestations that are not evident at the time of
our inspection. Our inspection does not include inspection of the
electric, heating, or mechanical systems of the structure. We did
not inspect the roof covering for leakage. Our inspection will
not detect building code violations. If any information is
desired about any of these mentioned areas, a company which makes
• home inspections should be engaged. The plumbing was inspected,
but only the leaks outlined in our report were found at this time.
We assume no responsibility for leaks that occur after the date of
this inspection. This structure has carpet installed. Although no
adverse conditions were evident, we assume no responsibility for
the floor covering under the carpet unless we remove the carpet.
The areas inside the soffitted eaves were not inspected. The
inaccessible areas listed above which were not inspected will be
inspected upon the owners request and may be conditional to
additional inspection fees.
NOTE: WORK PERFORMED BY OTHERS WILL BE REINSPECTED BY THIS
COMPANY FOR A REINSPECTION FEE OF $125.00. OPEN WALL AND OPEN
FLOOR INSPECTIONS ARE REQUIRED IF A CERTIFICATION IS DESIRED FROM
THIS COMPANY. REINSPECTION MUST BE PERFORMED WITHIN FOUR MONTHS OF
THE ORIGINAL INSPECTION DATE; SCHEDULING FOR REINSPECTION MAY
REQUIRE AS LONG AS TEN DAYS TO BE MADE.

noted to be heavily corroded. Active leakage was not evident
at this time.
RECOMMENDATION: The owner is advised to contact a licensed plumbing
contractor to replace the deteriorated fittings.

10B. FINDING: There is visible mold in the backing of the linoleum on the
downstairs hall bathroom slab floor.
RECOMMENDATION: As this is a cosmetic item only, a bid for replacement of
the floor covering in this area will be supplied upon
request.

10C. FINDING: The toilet in the downstairs hall bathroom is loose and
leaking.
RECOMMENDATION: Reinstall the toilet correctly using a new no -seep toilet
seal. If, upon removing the toilet, a cracked toilet or
• improper hercules ring is revealed, there would be
additional charges for a new toilet or repairs to the
hercules ring.

indicate leakage through the roof covering.
RECOMMENDATION: The owner should have a licensed roofing contractor check
and repair the roof covering as necessary.

10H. FINDING: The upstairs hall bathroom floor is fungus damaged adjacent
to the tub.
RECOMMENDATION: Remove the toilet and open the floor to repair damage as
required. Install new underlayment. Install new standard
grade, neutral colored, vinyl floor covering over the
• bathroom floor area. Reinstall the toilet using a new no -
seep toilet seal. When the underlayment is removed, if
fungus damage to the subfloor or walls is revealed, or a
cracked toilet or damaged hercules ring is found, we would
issue a supplemental report and bid.

10I. FINDING: Water stains were noted at the windows. This condition
appears to have been caused by condensation.
RECOMMENDATION: The owner is to keep these areas well sealed and as dry as
possible to prevent moisture intrusion.

tile on the upstairs master bathroom tub.
RECOMMENDATION: Regrout this area in the most practical way.

RECOMMENDATION:
Supply and install a new toilet assembly on a new no -seep
toilet seal.

toilet.
RECOMMENDATION:
Remove the toilet and open the floor to repair the damaged
underlayment as required. Install new underlayment, and

covering over the bathroom floor area. Reinstall the toilet
using a new no -seep toilet seal. If, when the underlayment
is removed, fungus damage to the subfloor or walls is
revealed, or a cracked toilet or damaged hercules ring is
found, we would issue a supplemental report and bid.

RECOMMENDATION:
Remove and replace the sink with a new stamped steel sink

AGREEMENT
HomeGuard Exterminators, Inc. is authorized to proceed with the work outlined in items
of their termite report no. 044177 for the property located at 11521 Sunrise Springs
Court, Cupertino 95014 fora total sum of $ This amount will be due and payable upon
completion of work. It is understood that the contract price does not include the charge of the structural pest control
inspection report or re -inspection fees.
HOMEGUARD EXTERMINATORS, INC. AGREES:
1. To guarantee all repair completed by this company for one year from the date of completion except for
plumbing, grouting, caulking, and resetting of toilets, which will be guaranteed for 30 days
2. To be bound to perform this work for the price quoted in our cost breakdown for a period rot to exceed 30 days.
3. To use reasonable care in the performance of our work but to assume no responsibility for damage to any
hidden pipes, wiring, or other facilities or to any, plant life, rain gutters, roofs; nor for damage or dirtying of
stucco, plaster, paint, wall paper or other "finish -work" adjacent to areas where work is performed.
OWNER OR OWNER'S AGENT AGREES:
1. To pay for services rendered including any additional services requested, upon completion of work. This
contract may canceled at any time by the customer. In the event of such action, customer agrees to pay
HomeGuard Inc. in full for any work already performed and the cast of materials and permits, plus 15% of the
total contract price to cover job set up and administration expenses.
2. To pay for service charge of 1.5 percent per month or portion of any month beyond 30 days after completion.
• 3. Owner grants HOMEGUARD, INC., a security interest in the above described real property to secure payment
of the sum for work and inspeciton fee completed.

1. If Additional damage is discovered by HOMEGUARD, INC., during the performance of work, company agrees

The charge for service that this company subcontracts to another person or entity may include the company's
charges for arranging and administering such services that are in addition to the direct costs associated with paying
the subcontractor. You may accept HomeGuard Exterminator's bid or you may contract directly with another
registered company licensed to perform the work
If you choose to contract directly with another registered company, HomeGuard Incorporated will not in any way be
responsible for any act or omission in the performance of work that you directly contract with another to perform.
Under the California Mechanics Lien law, any structural pest control operator who contracts to do work for you, any
contractor, subcontractor, laborer, supplier or other person who helps to improve your property, but is not paid for
work or supplies, has the right to enforce a claim against your property. This means that after a court hearing, your
property could be sold by a court officer and the proceeds of the sale used to satisfy the indebtedness. This can
happen even if you have paid your contractor in full if the subcontractor, laborers of suppliers remain unpaid. To
preserve their right to file a claim of lien against your property, certain claimants such as subcontractors and material
suppliers are required to provide you with a document intitled "Preliminary Notice". General contractors and laborers
for wages do not have to provide this notice. A Preliminary Notice is not a lien against your property. Its purpose is
to notify you of persons who may have a right to file a lein against your property if they are not paid. You can protect
yourself from such claim by requiring your contractor to supply you with a payment and performance bond prior to
. commencing any work of improvement and/or requiring your contractor to provide you with an unconditional lien
release signed by each material supplier, subcontractor, and laborer involved in that project phase before making
payment on the completed phase of the project.

HomeGuard Exterminators, Inc. is authorized to proceed with the work outlined In items
of their termite report no. 044177 for the property located at 11521 Sunrise Springs
Court, Cupertino 95014 fora total sum of $ This amount will be due and payable upon
completion of work. It Is understood that the contract price does not Include the charge of the Structural pest control
inspection report or re -inspection fees.
HOMEGUARD EXTERMINATORS, INC. AGREES:
1. To guarantee all repair completed by this company for one year from the date of completion except for
plumbing, grouting, caulking, and resetting of toilets, which will be guaranteed for 30 days.
2. To be bound to perform this work for the price quoted in our cost breakdown for a period not to exceed 30 days.
3. To use reasonable care in the performance of our work but to assume n0 responsibility for damage to any
hidden pipes, wiring, or other facilities or to any, plant life, rain gutters, roofs: nor for damage or dirtying of
stucco, plaster, paint, wall paper or other "finish -work" adjacent to areas where work is performed.
OWNER OR OWNER'S AGENT AGREES:
1. To pay for services rendered including any additional services requested, upon completion of work. This
contract may canceled at any time by the customer. In the event of such action, customer agrees to pay
HomeGuard Inc. in full for any work already performed and the cost of materials and permits, plus 15% of the
total contract price to cover job set up and administration expenses.
2. To pay for service charge of 1.5 percent per month or portion of any month beyond 30 days after completion.

responsible for any act or omission in the performance of work that you directly contract with another to perform.
Under the California Mechanics Lien law, any structural pest control operator who contracts to do work for you, any
contractor, subcontractor, laborer, supplier or other person who helps to improve your property, but is not paid for
work or supplies, has the right to enforce a claim against your property. This means that after a court hearing, your
property could be sold by a court officer and the proceeds of the sale used to satisfy the indebtedness. This can
happen even if you have paid your contractor in full if the subcontractor, laborers of suppliers remain unpaid. To
preserve their right to file a claim of lien against your property, certain claimants such as subcontractors and material
esuppliers are required to provide you with a document intitled "Preliminary Notice". General contractors and laborers
for wages do not have to provide this notice. A Preliminary Notice Is not a lien against your property. Its purpose is

Further inspection was performed as recommended in items 10A, 10G,
10N, and 11H, and the following was noted:
(2) STALL SHOWERS
2A. FINDING: The shower downstairs was retested after opening the
sheetrock (item 10A) and leakage and fungus was noted.
RECOMMENDATION: Break out and remove the entire existing stall shower.
Remove and replace the damaged wood members. Install a new
waterproof pan membrane. Install an approved surface on the
walls. Install new ceramic tile. If damage is found to
extend into inaccessible areas, a supplemental report will
• be issued with any findings, recommendations, and bids.

NOTE: It may be necessary to remove the mirror and vanity top to
effect the repairs.
2B. FINDING: The glass shower enclosure in the downstairs bathroom is not
tempered glass and the local building department will not
allow this unit to be reinstalled.
RECOMMENDATION: Remove and replace this shower enclosure.

10P. FINDING: There is a plumbing leak at the upstairs hall bathroom tub
drain.
RECOMMENDATION: Reseal or repair this leak in the most practical way.

10Q. FINDING: The 2x4 stud has been damaged by fungus adjacent to the
upstairs hall bathroom tub.
RECOMMENDATION: Remove and replace all damaged wood members. If any damage
is found to extend into any enclosed areas a supplemental
report and bid will be issued.

• NOTE:'We intend to open the sheetrock at the adjacent bedroom
closet to effect the repairs outlined in items 10M and 10N.

10S. FINDING: Water is leaking out around the master bathroom glass shower
enclosure.
RECOMMENDATION: Reseal this enclosure in the most practical way. With this
type of enclosure, we cannot guarantee to completely stop
the leakage, only to control it. It will be the owners
responsibility to maintain this enclosure in a waterproof
condition.
